Reports off to triage and primary Hospice team concerning patient related visit, changes to plan of care, and follow up needs. Follows NYS Standards for Professional Registered Nurse. Graduate from an accredited School of Nursing. At least two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing preferred.
