Job summary: Experienced Peer Specialist to work on a newly funded innovative Care Transition team that serves participants transitioning to the community from a higher level of care. As a member of the Pathway Home multi-disciplinary team the Peer Specialist position will work under the supervision of the Team Leader. The role will involve facilitating transition from one level of care to the community by addressing the preparatory needs of participants in the early stages of recovery, discharge planning, development of daily living skills, and coping mechanisms through team and individual work in the community. The team will follow individuals into the community using the Pathway Home model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Peer Specialist engages participants in peer-based services to identify and overcome barriers to community integration, utilizing a peer relationship to foster connection, trust, understanding and validation with community providers and supports. The role will require on call coverage and a willingness to work flexible hours. Job Responsibilities: • Engagement beginning from referring location with involvement in transition planning, including a needs assessment of community transition supports essential to stabilizing the participant;, • Individual sessions with participants emphasizing prevention and preparing for independent community living and the promotion of optimum mental and emotional health. Position Overview: HEAF is seeking compassionate, energetic, and organized SEL & Wellness Coaches to support social-emotional learning (SEL), wellness programming, and student, family, and community engagement initiatives within the Community Wellness Department. This hands-on, relationship-centered role helps students and families feel connected, empowered, and emotionally supported while guiding students to build resilience, recognize their strengths, and thrive. SEL & Wellness Coaches will facilitate afterschool SEL & Wellness classes through HEAF Achieve, support school-day SEL, wellness, and academic workshops through HEAF Inspire, and contribute to the Youth Leadership & Ambassador Program. The Department of Psychiatry is seeking psychologists assigned to work in the CUIMC FPO. These are full-time, clinical positions in the Columbia Day Program. Primary responsibilities involve conducting group therapy utilizing empirically supported treatments, diagnostic/intake evaluations of adults, and individual treatment planning and progress monitoring with patients. Candidates should have previous experience providing outpatient treatment and working with a multi-disciplinary team. Experience and training in Dialectical Behavior Therapy (DBT) and Cognitive Behavioral Therapy (CBT) preferred. Candidates with additional experience in evidence-supported treatment delivery for trauma and/or substance use are desired. Ideal applicants will have experience, familiarity and comfort working with high-risk patients, as well as young adults and college students. Applicants should have strong interpersonal skills and the ability to work well across disciplines and function effectively as part of a multi disciplinary team.
