Responsibilities include: (Detailed Task) Clinical Responsibilities: • Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools., • Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes., • Deliver clinical consultation and guidance related to infectious diseases to site medical directors (SMD), infection control teams, and clinical staff. The Quality Assurance (QA) Radiological Technologist position encompasses responsible, administrative and clinical activity of varying degrees of latitude. Clinical level judgement is needed in the daily supervision of radiological functions and patient care, and departmental projects. Responsibilities include: • Review exam orders, consult with radiologist, schedule patients and arrange for transportation as indicated., • Identify patient and document all information pertinent to final report in the RIS and on images (PACS)., • Prepare all patients (infants, children, adolescents, adults and geriatric) for radiological procedures requested by the clinician. The Department is comprised of 70+ healthcare professionals who are responsible for providing re-entry services to individuals detained in the NYC jail system. The Social Work Department supports patients’ rights and criminal-legal advocacy efforts in collaboration with patients’ criminal defense attorneys, Alternative to Incarceration (ATI) programs, and other post-release supervision providers. Additionally, our Social Workers coordinate closely with the New York State Office of Mental Health (OMH) and the NYC Department of Health & Mental Health (DOHMH) to provide robust services to our most vulnerable patients. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Nurse Practitioner will be matched with the population that most closely meets their clinical expertise, desired work location and schedule, and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs, • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization, and escalate care when clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care., • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. Detailed Tasks: • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. Outposted Therapeutic Housing Units Program (OTxHU) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient’s clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management. Conduct routine safety rounds in all facilities where CHS employees are located while identifying potential environmental safety hazards to the Department of Corrections (DOC) for mitigation. The Occupational Safety & Environmental Health Coordinator will engage with staff to identify safety gaps or environmental health concerns, provide on-going support and follow-up for staff involved in workplace violence incidents and/or who have suffered occupational injuries. This position serves as a central resource for safety education, incident response, and environmental risk mitigation across all CHS sites and services. Correctional Health Services The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs., • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care., • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Safety • Enforce company safety policies and site-specific safety plans., • Conduct daily safety walks and weekly toolbox talks., • Identify and correct unsafe conditions or behaviors on site., • Ensure compliance with OSHA regulations and maintain safe separation and protection for occupied areas, tenants, and the public where applicable. Quality Control • Inspect work in progress to ensure it meets project specifications and quality standards., • Identify and document existing-condition issues uncovered during renovation work., • Coordinate testing and inspections with third-party inspectors and local building department officials., • Develop and track punch lists through project closeout and turnover. Coordination with Project Team • Collaborate with the Project Manager, architects, and engineers to plan work activities., • Communicate field conditions, progress updates, and issues impacting cost or schedule., • Assist with RFIs, submittals, and resolving design conflicts or unforeseen existing conditions., • Support progress reporting, as-built documentation, and closeout documents.
