Required Documents
1. U.S. Passport, Permanent Resident Card, or Valid Work Visa
o If expired, please bring the extension letter or renewal documentation.
2. Social Security Card
3. State-Issued Photo ID
4. HHA or PCA Certificate
5. Two (2) Professional Reference Letters
o Must be written in English only
6. Complete Physical Exam (Required)
o Must be no older than one (1) year
o Must be fully completed, signed, and stamped by the provider
o Include the following lab results:
• Rubella and Rubeola (Measles) Titers
• Drug Screen
• PPD (Tuberculosis Test) – must be no older than 3 months
• If the PPD result is positive, a Chest X-Ray is required
