Monitor patients’ vital signs and observe for any changes in health status, documenting findings accurately to inform care plans. Assist with medication administration following HIPAA guidelines and facility protocols, ensuring safety and compliance at all times. Support residents with meal preparation, feeding, and nutritional needs while respecting dietary restrictions and preferences. Maintain a clean and safe environment by performing light housekeeping duties such as cleaning patient areas, laundry, and organizing personal belongings. Facilitate effective communication with clients in both English and Spanish to ensure understanding of care procedures, preferences, and concerns. Support residents’ rights by respecting their autonomy, dignity, and cultural backgrounds while providing compassionate care tailored to individual needs. Qualifications Proven experience working as a caregiver in group homes, assisted living facilities, nursing homes, or home health environments. Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, and neurodevelopmental disorders such as autism or intellectual disabilities. Familiarity with patient observation techniques, patient monitoring tools, and developing or following detailed care plans. Ability to perform heavy lifting safely using proper manual handling techniques; experience with Hoyer lifts is preferred. Certification in CPR and First Aid is required; additional certifications related to healthcare or caregiving are advantageous. Strong communication skills in both English and Spanish; ability to effectively engage with diverse populations. Understanding of resident’s rights, HIPAA privacy regulations, and home & community care standards.     We are looking for both English/Spanish Care Coordinators Job Summary: The Home Care Coordinator is responsible for coordinating and managing home care services to ensure clients receive high-quality, compassionate, and individualized care. This role involves assessing client needs, developing care plans, scheduling caregivers, monitoring service delivery, maintaining accurate documentation, and communicating with clients, families, and healthcare professionals. Key Responsibilities: - We have our own system we use (We will train you for this part) - Pre-Billing - Act as a direct link between family, the home care agency, and the insurance case manager. - Communicate with patients and aides - Ability to staff cases - Problem solver / Good people skills - Communicate with MLTC to authorize services, adjust care hours, report changes in patient health, and updating care plans. Qualifications: • Experience in healthcare administration, nursing, social work, or a related field preferred, • Experience in home care, healthcare coordination, case management, or patient services a must, • Knowledge of home care regulations and documentation requirements., • Strong organizational, problem-solving, and multitasking abilities., • Excellent communication and interpersonal skills., • Proficiency with electronic health records (EHR) and Microsoft Office applications., • Ability to work independently and collaboratively in a fast-paced environment.
