Principal Responsibilities • Demonstrates fidelity to the agency’s service values and organizational imperatives., • Provides aftercare support to cases transitioning to trial discharge, using MST-FIT model of treatment/ therapy., • As necessary, provides additional support to cases transitioning from trial discharge MST-FIT to return to placement, • Provides crisis intervention and conflict resolution to youths and their families throughout the week, to include evenings/weekends as required but minimally on a consistent rotation schedule., • Conducts virtual and in person sessions to resident youth/families., • Responsible for, at least, weekly documentation to include case, collateral and other contacts w/I 48 hours of occurrence., • Ensures on-going support and collaboration across MST and ITM program/ staff on behalf of residents., • Prepares and submits MST-FIT engagement court reports for scheduled permanency hearings., • Prepares for and attends weekly and other ITM learning community meetings and trainings., • Attends monthly/ mandatory trainings to include 7-day ITM, 5-day MST-FIT booster trainings., • Responsible to prepare for and attend weekly supervision and MST-FIT group consultations as scheduled., • Must prepare for and attend monthly MST-FIT psychiatric consultation as scheduled., • Supports the social service department with regard to resident transition by way of MST-FIT model of treatment engagement Required Qualifications and Experience • Minimum MSW degree with licensure preferred., • Minimum of 1 year of experience working in child welfare with at risk youth., • Former Case Management experience is a plus., • Valid NYS Driver's License, • Professional, mature and dependable with a bias for action., • Ability to assess and engage youth and families is critical to the position., • Physical ability to travel throughout the community, securing resources and services, while adhering to the MST 120-day targeted length of treatment model., • Willingness to adjust schedule as needed to ensure service needs are met. Travel Requirements This position offers a full-time, hybrid/in field schedule and is based in Rockaway and does require home visits and travel to other locations. Job DescriptionSummary Job Description The Assistant Director of Social Services provides programmatic and administrative leadership to the Social Services Department within the Residential Treatment Center (RTC). The Assistant Director assists in overseeing case planning, permanency planning, family engagement, discharge planning, documentation, regulatory compliance, and supervisory functions to ensure the delivery of high-quality, trauma-informed, family-centered services that support positive outcomes for youth. The position works collaboratively with Residential, Medical, Behavioral Health, Quality Improvement, and external stakeholders including OCFS, ACS, Family Court, and community providers. Principle ResponsibilitiesDepartmental Leadership • Assist in directing the day-to-day operations of the Social Services Department., • Provide supervision and guidance and support to Social Services staff to ensure quality case practice., • Promote practices that advance positive outcomes in permanency, well-being, and safety for youth., • Ensure comprehensive assessments, service plans, progress notes, and case documentation are completed timely and meet agency, ACS, and OCFS standards., • Review and approve Family Assessment and Service Plans (FASPs), Permanency Hearing and Court reports, discharge plans, and other required documentation., • Support staff in managing complex behavioral, family, permanency, and crisis situations., • Promote trauma-informed, culturally responsive, and family-centered practice., • Collaborate with multidisciplinary teams to address the individual needs of youth and families. Permanency Planning • Monitor permanency outcomes and ensure timely movement toward reunification, kinship, adoption, APPLA, or other approved permanency goals., • Ensure meaningful family engagement throughout placement., • Monitor visitation plans and family contact., • Participate in permanency conferences and Family Team Conferences., • Remove barriers delaying permanency. Regulatory Compliance • Ensure compliance with:, • NYS OCFS regulations, • ACS Quality Assurance Standards, • Medicaid/29-I documentation requirements (if applicable), • Justice Center and Critical Incident reporting requirements, • Agency policy and procedures, • COA Standards, • Monitor case records for quality and timeliness., • Prepare for internal and external audits., • Assist in corrective action planning. Establishes/over-sees the application process., • On the job safety concerns., • In conjunction with departmental supervisors, offers on-the-job guidance, direction and support for our in-house youth workforce., • Provides job readiness guidance and training for all RTC youth as well as future-oriented vocational guidance and support, in collaboration with teachers, permanency planners and Youth Counseling staff., • Serves as the agency's primary youth "Job Developer" (i.e., develops relationships with potential community-based employers to identify and match job opportunities with our youth, as appropriate)., • Maintains job schedules for all youth workers. Keeps current on our youth's job performance (both in-house and off-grounds jobs)., • Counsels and documents accordingly., • Develops and maintains an on-grounds vocational library., • Works to develop and maintain strong, caring, therapeutic relationships with all RTC youth., • Attends treatment meetings as required., • Assists in other Departmental program activities as assigned by supervisor., • Works with young people who are aging out to submit all forms of housing applications, as early as possible, • Checks in on young people’s application status; follows-up with NYCHA and supportive housing residences, when needed, • Helps young people transition into permanent housing, when applicable, by helping them obtain Section 8 vouchers, apply to the NYC affordable housing lottery, etc., • Liaises with the Case Planner and Coach, as needed. Required Qualifications and Experience, • Bachelor’s degree required, Psychology, Social Work or related field preferred, • One year experience in a Child Care agency/ Child welfare system, and experience working with educational programs preferred, • Clean NYS driver’s license and own personal car preferred to transport residents to court and other appointments., • Direct experience working with troubled youth., • Knowledge of employment, career exploration, and vocational training opportunities that support positive outcomes for youth.
